1st  ANNUAL VERMILLION AREA SOCCER TOURNAMENT

Tournament Packet

If you are a club team, these are the items needed to participate in the
1st Annual Vermillion Area Soccer Tournament:

• Tournament Application Form ~ received by 9/2/2011. (Word Doc Fill in and print)
• Medical Release Form ~ one per player.
A Traveling Player Form ~ can be downloaded at

• If not from South Dakota, traveling papers must be presented.
• 2 Copies of Your Team Roster.
• Copy of each player’s Birth Certificate.
• Player Cards.
